Authentication

Authentication is handled at the gateway level. The portal trusts headers from the reverse proxy:

Authorization — Bearer token (JWT)
X-User — User identifier
X-Role — Role (patient, provider, partner, admin, os)
X-Tenant — Tenant/organization identifier
